Requirements for the program:

Must have Medicare Part A & B (no Medicare Advantage)
Must have a diagnosis of mild, moderate or severe dementia

If you or a loved one have been diagnosed with mild, moderate, or severe dementia, you may be eligible for support through the federal government’s new program, Guiding an Improved Dementia Experience (GUIDE).

Care Dimensions has been selected by the Centers for Medicare and Medicaid Services to provide GUIDE services that aim to:

Improve quality of life for people living with dementia
Reduce strain on their unpaid caregivers
Enable people living with dementia to remain in their homes and communities as long as possible
